At Brookvale Dental Practice, we aim to provide high quality dental care in an environment where you will feel calm, confident and safe. We offer up-to-the-minute dentistry with an emphasis on preventative techniques – helping you to understand how to maintain the health of your mouth and keep it free from tooth decay and gum disease.
Therefore, we are pleased to offer you the opportunity to join our Membership Plan. We believe this will enable you to spread the cost of routine care and will provide you with the best chance of staying dentally healthy and minimise the need for any treatment.

What does the Membership Plan cover?
- Two detailed dental examinations per year

What does the Membership Plan cost?
The fee for the membership plan is £15.30 per month, usually payable by direct debit although it can be paid annually in advance if you prefer.
How do I join?
Joining the plan is very simple. All you have to do is complete a direct debit mandate and registration form and your cover will be effective from the start of the next month. If you decide at any time to end the agreement you can do so simply by giving us one month’s notice. Naturally, if amounts due to us are not paid for two months in a row, we may terminate the arrangement.
What happens if I have an emergency?
If you have an unexpected emergency whilst at home, such as an accident or toothache, then in normal circumstances you should ring our emergency number. (01404 44800)
If you are away from home, membership of our plan allows you to call the dental helpline who will put you in touch with a dentist who can help. (UK) 0800 525631

How do I claim under the cover?
If you are away from home and have to see a dentist in an emergency, you should receive the temporary treatment necessary to render you in a comfortable condition. You pay that dentist and ask for a written receipt and details of treatment carried out. On your return home, we will help you complete a claim form and forward this to DPAS, who administer our plan.
We will also help you fill in a claim form should you need treatment for a dental injury. If the treatment is likely to cost more than £175 (and thus require prior authorisation from DPAS) we will provide a costed treatment plan. An assessment by an independent dentist may be requested before this permanent treatment is authorised.
